The MSF is a 501©(3) organization as recognized by the Internal Revenue Service and is registered with all appropriate state authorities. As such, donations are tax deductible to the fullest extent allowed by law. Consult your tax advisor for applicability.

For donations via check or money order, either send the detached stub from the pledge confirmation you received in the mail or complete and print a copy of this form and mail along with your check or money order to:
